Instructions: Fill out the form as completely as possible.

For OFFICIAL transcripts you must provide the address for each individual educational institution that you would like your transcripts forwarded to. Upon hitting submit you will be transferred to a secure payment area (PayPal) in order to make your $25.00 per transcript payment towards the processing of the transcript. You will be required to pay this fee for each transcript request.

NOTICE! This form is NOT for obtaining transcripts from an institution you attended other than Nation -2- Nation Christian University. Please contact the institution(s) you attended to obtain your official transcript from them.
